A musty smell in a carpeted room is a symptom, not a problem in itself. It means there is enough moisture in the floor structure for mould or bacteria to be actively growing. Deodorising the room deals with the symptom and leaves the cause untouched, which is why the smell always comes back.

Where the moisture is coming from
There are only a handful of realistic sources, and identifying yours determines everything that follows.
Subfloor moisture. Very common in older piled houses. Damp ground under the house evaporates upward through the floorboards into the carpet and underlay. Usually affects a whole room or the whole house evenly.
Rising damp or slab moisture. In houses on concrete without a proper vapour barrier, moisture moves up through the slab. Often worst around the edges of a room.
Condensation. Warm moist indoor air meeting cold surfaces. Tends to be worst in bedrooms, especially where windows stream in the mornings.
A leak. A slow plumbing leak, a failed shower seal or a roof problem. Usually localised, and often smells strongest in one specific patch.

How to check without special equipment

Look at the back of the carpet for dark grey or black speckling, which is mould
Check the subfloor if you can access it. Damp soil, standing water or blocked vents are all diagnostic

Note whether the smell is worse after rain, which points to subfloor or leak, or worse in the morning, which points to condensation

Fixing the cause
This is the part people skip, and it is the only part that produces a lasting result.

Check that downpipes and ground levels are moving water away from the house rather than under it.
Then deal with the carpet

A thorough job involves an antimicrobial treatment, hot water extraction with strong recovery, replacement of any contaminated underlay, and forced-air drying afterwards. Done properly, a musty room can be genuinely fresh within a day.
Done badly, by adding water without recovering it, the smell gets worse.
